Why is the survey required?
As an artisan insurance policyholder, you are required to periodically answer questions about your business.
When does the survey need to be completed?
In general, new policyholders must complete the survey within the first few weeks of the beginning of your policy.
For current policyholders, this survey must be done before the policy can be renewed. Please refer to the letter
you received for more information.
What do I need to complete the online survey?
- The survey notification letter from Utica First Insurance
- Your policy number
- The ZIP Code on the policy's mailing address
- A valid email address
- A compatible web browser
- Internet Explorer 6.0 or better
- Mozilla Firefox 3.0 or better
- Adobe Reader
What happens if I step away from the computer while taking the survey?
For your protection and privacy, if you are away from the computer for more than 45 minutes, your survey session will time out.
Please make sure you have completed and submitted the survey.
At the end of the survey there is a Submit Survey to Utica First button. Your survey is completed only after clicking this button. Shortly after
submitting your survey, you will receive a verification email. This email is your receipt for completion of the required survey. Please keep it for your records.
You must click the button to submit your survey. If the survey is not submitted, it will be considered incomplete and may be subject to cancellation. Please call the Utica First
Artisan Survey Unit at 1-800-456-4556 Option 1 if you do not receive a verification e-mail.
